for Existing Patients

  • Even with this virtual appointment, some in-person appointments will still be needed.

  • Ortho South is not responsible for any data charges that may occur using the virtual evaluation.

You will need 2 spoons to hold your cheeks out of the way. It’s also recommended that you have someone help you take photos. Be sure to use flash on your camera to help light your teeth and mouth. The images below are examples of how to take each photo. You will be able to upload all of your photos at the end of the form.


IMPORTANT

If you are in aligners (invisalign® or shift™) you MUST upload 2 versions of each photo. One photo with your aligners IN (and elastics, if applicable) and
one photo with your aligners OUT.

    Bite down on your back teeth. Pull both cheeks back as far and wide as you can to show as much of your teeth and gums as possible. Snap photo directly in front of your mouth.

    Pull both cheeks back as far and wide as you can to show as much of your teeth and gums as possible. Open your mouth just a little to add space between upper and lower teeth. Snap photo directly in front of your mouth.

    Bite down on your back teeth. Use the spoon to pull back your right cheek as far as you can. Snap photo directly at the right side of your mouth.
